How long will it take me to get an appointment?

Usually same day appointments.

 

What is the typical wait time?

Usually 5 to 10 min.

What do you charge for Co-Pay?

For insured patients we collect normal Physician office (Primary Care) standrad co-pay for any visit. We do not collect Urgent Care co-pays that are usually high and we do not charge any convenience fee for weekend and evening hours like other doctor's offices. We keep it simple and provide excellent medical care at lowest cost to you.

When should I have a complete physical?

 You should have one once a year, every year.

What days are you open?

 Our office is open 7 days week.  

What services do you provide?

We provide Primary Care and Urgent Care services for all individuals and families.  

Can I get a copy of my lab results?

Yes, we will be glad to give you results. We can make these results available for you online in our online Windermere Patient Portal  or Fax or pick up at our location. Please contact us for details.

Our billing office can be reached at  (678) 867-7630 Monday to Friday 9am to 5pm.

 

What is your Medication Policy?

It is the policy of our office to not phone in ANY controlled substance to your pharmacy.  Maintenance medications will not be phoned in unless you have been seen in this office in the last six Months. Please allow 24-48 hrs on any maintenance medications to be refilled upon notification in this office. Our goal is to provide the best medical care to meet your needs and this policy is in place to adhere to the high medical care standards we strive to provide.

 

What is your Forms Policy?

It is the policy of this office to charge a $20.00 fee for each occurrence of any medical forms needing to be completed. (i.e. disability forms, physical forms, insurance forms, etc.)
