If you have a HMO or Medicaid plan that requires a Primary Care Physican to be printed on your card, then please call your insurance to change your PCP to us so that your visits will be covered by your insurance.
We are communicating with other medical insurance companies to become listed as a "In-Network" health care provider. If your insurance company is not listed, please call our office to see if it has been recently added. Also, It still may be possible to utilize our practice as an out of network provider.
If you do not have any insurance, we provide reasonble self pay rates. Also, please enquire us about self pay plans for individual and family with no insurance or high deductable insurance plans.
