The blood work requirements include subject to age:

  • T.B. Gold test (in case it comes positive, then you will be required to perform an X-ray)
  • Syphilis (in case it comes positive, then additional confirmatory test to order)
  • Gonorrhea
     

Items to Bring For the Immigration Medical Exam

Please note that there are certain requirements by law that you must fulfill when you come to our clinic for the immigration medical examination. This will help us proceed without any delay.

The items that you must carry along with you on the day of the examination are listed below:

  • Please fill out the I693 form in the office (including the name and A# on each page if available).
  • Bring your original Passport or any other government-issued photo I.D. such as Driver’s License, State I.D., Consular I.D., or School I.D. Also, make sure that they are current/valid.
  • Proof of your vaccinations if available (Translated to English if necessary).
  • Immunization Records if any.
  • T.B. treatment records if any.
  • Prior mental health records if any.
